GENERAL SUMMARY: Provides leadership for program development and system-wide implementation of Advance Care Planning (ACP) across health system in order to meet clinical and strategic objectives including: a) establishing the clinical and business case for ACP advancement, b) performance monitoring and measurement of program initiatives, c) reporting of program progress, and d) analyzing and critically evaluating program growth opportunities. Responsible for coordinating activities of Advance Care Planning, identifying appropriate resources needed, and training appropriate individuals in First, Next, and/or Last Steps Advance Care Planning.
Establishes and leads an ACP Steering Committee to monitor and evaluate the ACP program being developed and implemented; serves as the key coordinator and administrative contact for day-to-day operations and execution of ACP program related activities and responsibilities.
Develops and builds the ACP infrastructure through strong relationships with leadership, physician and community partners in order to advance the program across the entire health system and the community. Partners with organizational leaders to develop and implement successful communications, EMR, quality improvement, and analytics requirements.
Ensures the achievement of ACP goals which include, but are not limited to, monitoring, measuring and reporting on progress, and proactively addressing issues to keep the program on track. Identifies problems, gaps, or issues with systems and analyzes, interprets, advises and consults with staff and other healthcare professionals to maintain high quality care.
Develops and maintains effective communication vehicles such as an ACP website, marketing and education materials, etc., to promote education and program growth.
Oversees research activities related to ACP.
Ensures ongoing competencies of RC certified facilitators, instructors and, over time, possible additional Organizational Faculty staff to ensure adherence to standardized, facilitated conversation.
Ensures ACP infrastructure requirements are in place including education, workflows, documentation, measurement, and reporting.
Leads faculty for mentoring and training of Respecting Choices certified instructors and additional ACP Organization faculty hired as part of the ACP implementation.
Engages the community in advance care planning, by working with community groups, professional organizations, consumer advisors and volunteers to raise awareness and promote advance care planning. Acts as an advisor to staff, patients, families and other community partners on ACP systems, protocols, and ethical guidelines as needed.
Identifies needs of the ACP workforce within the system and supports the team in their development and ongoing confidence and competence.

Education / Training: Master's degree in nursing or social work. Physicians or chaplains often fill this role as well.
Work Experience: 5-7 years progressive healthcare leadership and management experience; Clinical ACP and end-of-life care experience preferred; project management experience strongly preferred. A track record of being results oriented, with demonstrated ability to drive performance is preferred with an ability to measure an interpret results. Demonstrated ability to prioritize work and be highly organized; excellent relational skills; ability to work effectively with diverse stakeholders across many care settings. Working knowledge of key elements necessary for being a change agent, and assuring long term dissemination and sustainability for organizational growth. Excellent computer and data management skills. Experience in research, quality improvement, and ACP preferred.
Certification, Licensure, Registration: If a nurse or social worker, current state license required. Clinical nurse specialist, adult/family nurse practitioner, doctoral prepared clinician. BSN/MSN, or Licensed Medical Social Worker. Chaplains must be CPE trained, with clinical chaplaincy experience.
Other Qualifications: ACP Facilitator and Instructor certification required within 9-12 months of hire. Respecting Choices Organizational Faculty requirements by the end of contracted services with Respecting Choices.
