Pomona Valley Hospital Medical Center (PVHMC) is a 437-bed, fully accredited, acute care hospital serving eastern Los Angeles and western San Bernardino counties.
The nationally recognized, not-for-profit Hospital is home to four Centers of Excellence: The Robert and Beverly Lewis Family Cancer Care Center, Stead Heart and Vascular Center, Women's & Children's Center, and Regional Kidney Stone Center, and offers a full-service Emergency Department, Neonatal Intensive Care Unit and Perinatal Center. Specialized services include sleep disorder diagnosis and treatment, robotic surgery, and the Family Medicine Residency Program, in affiliation with the University of California, Los Angeles.
Position summary: Manages high risk patient care, including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting, including a registry. Participates on a team for data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the Patient Centered Medical Home. May perform other duties as assigned
Job Qualifications [Required] Valid unrestricted California Registered Nurse. Bachelor's degree in Nursing. BLS. Two+ years of experience in a clinical setting. Able to maintain confidentiality with all aspects of information in accordance with practice, State and Federal regulations. Ability to communicate and outreach to other community health care organizations and personnel. Must be bilingual in English and Spanish. Proficient computer skills, including Microsoft Office (specifically Word and Excel). Highly organized with well developed oral and written communication skills. Experience with electronic medical records.
[Preferred] Master's degree in Nursing. Two+ years of experience in chronic disease management, case management, utilization management and adult acute care. One year experience in Patient Centered Medical Home clinical setting or knowledge of the Patient Centered Medical Home. Bilingual Spanish speaking and proficient in Spanish reading and writing skills. Experience with public speaking.
