The Jefferson College of Population Health at Thomas Jefferson University (Philadelphia, PA) is recruiting a strong operational and visionary leader to serve as the inaugural Executive Director of a new community-based initiative designed to improve population health in Somerset and Cambria Counties in Western Pennsylvania.
The community-based initiative is based on the successful Pathways Community HUB model (https://pchi-hub.com), which has been replicated in more than forty communities in the United States. The new Executive Director will have the opportunity to completely develop and implement the multi-year initiative in collaboration with consultants, vendors, and key staff, and with the support and oversight of the 1889 Foundation and the Jefferson College of Population Health at Thomas Jefferson University.
The Executive Director will oversee all aspects of the initiative, including:
engaging and partnering with community care coordination agencies serving at-risk populations;
developing and maintaining relationships with diverse stakeholders, including care coordination agency members, community members, referral partners, providers, and payers;
developing and managing contractual relationships with payers;
developing and managing performance outcomes and contractual compliance;
initiating and overseeing vendor contracts to support program implementation;
managing professional and administrative personnel;
overseeing activity-based budgets;
establishing and overseeing the Community Advisory Board;
ensuring that all policies and procedures are in place and updated as needed; and
reporting progress and results to foundation board members and University staff.
The ideal candidate will be a leader in community health improvement with a history of working successfully across multiple stakeholders to accomplish population-level impact. She or he will have strong planning and management skills necessary to develop and implement large scale community improvement efforts with careful attention to detail, while at the same time possessing effective leadership skills that inspire confidence through clear communication of vision, thoughtful and measured action, and a results-driven ethos. He or she should also have experience leveraging analytics and technology that inform care management and care coordination.
In addition, the ideal candidate will have:
experience working with health insurance companies and/or health/social service provider organizations;
exposure to value-based and alternative reimbursement contracting;
experience with initiatives that extend clinical services beyond traditional post-discharge care coordination;
experience implementing programs that help to address social determinants of health;
history of significant budget and staff oversight responsibilities;
track record of designing, implementing, and evaluating population-level health improvement programs;
strong record of building and managing a team, while also operating at a high strategic level;
graduate degree in public health, management, nursing, health administration or relevant area; and
experience in senior-level management that includes budget responsibilities, personnel management, coalition leadership, and effective engagement with community oversight boards.
This position is not eligible for telecommuting and requires some travel (mostly local). The candidate may qualify for a faculty appointment in the Jefferson College of Population Health.
The Executive Director will have a direct reporting relationship to the senior leadership of the Jefferson College of Population Health. The Executive Director will also engage with leadership and board members of the 1889 Foundation.
Compensation and Relocation
The Executive Director will receive a competitive salary and benefits commensurate with a senior administrator position at Thomas Jefferson University, which include medical, dental, and vision insurance; several retirement options with employer matching; subsidized tuition; and others. Relocation and moving expenses will also be included in the package.
