The Patient Safety Process Manager’s primary site of work is Reading Hospital but supports individuals across Tower Health to successfully utilize RL Solutions for reporting, investigating events and managing the system across all sites. The Patient Safety Process Manager acts as an organizational resource for patient safety concepts, tools and methods. Primary responsibilities include facilitating and advising on Cause Analysis methods to identify causes and contributing factors for safety events and near misses. This individual also facilitates Failure Modes and Effects Analysis and Apparent Cause Analysis Teams. Work with key leaders to assure that corrective actions are addressed, and improvement monitored in response to what is learned in the analysis. Engage with frontline staff, staff responsible for event reporting and is active in the electronic reporting system. Advocates for/supports improvement efforts and is a resource for clinical regulatory compliance. The person in this role performs all the above in collaboration with others in the Patient Safety and Risk Management Structure and others in the TH community as appropriate for the situation. They are knowledgeable about both RL Solutions as well as MCARE and reporting expectations for the DOH and PA Safety Authority. They assign Harm Scores and support a process to review and ensure a standard approach to the use of Harm Scores across the facility.
BSN/BA/BS or bachelor’s Degree in related healthcare profession required. Master’s preferred.
Direct experience with clinical process improvement, process reengineering, healthcare quality improvement or like clinical initiatives preferred.
Experience in implementation of regulatory Joint Commission initiatives and/or Joint Commission National Patient Safety Goals preferred.
Current CPPS certification or obtainment within two years of employment
Minimum of 5 years of work experience in healthcare and understanding of care across the continuum
Demonstrated experience managing cross-functional clinical teams, including physician groups, broad clinician groups as well as consultants and contractors. Strong interpersonal, verbal and written communications skills are required for the successful management of this function. Ability to prioritize work, manage time and demonstrate excellent organizational skills and initiative to improve processes.
