Have successfully completed training in an ACGME-accredited residency program in Anesthesiology, Emergency Medicine, Family Medicine, Internal Medicine, Neurology, Obstetrics and Gynecology, Pediatrics, Physical Medicine and Rehabilitation, Psychiatry, Radiation Oncology or Surgery Be board-certified or board-eligible in one of the above mentioned specialty areas. Currently have or are able to obtain a license to practice medicine in New York prior to the date the fellowship is scheduled to begin. A recent photograph Curriculum Vitae (must be month/year specific) A copy of your current state medical license (if applicable) A copy of your current DEA certificate A copy of your USMLE, Comlex and/or National Board Certificate A notarized copy of your Medical Diploma Certified medical school transcript Copy of all board certification certificates A personal statement, which outlines your reason for pursuing a career in palliative medicine, your experiences with palliative care, and your career goals. Three letters of reference (One from your current or prior Program Director).
